**This is the Patient Portal for Mercy Medical Center in Roseburg Oregon**
Effective 10/2/23, we have migrated to a new Patient Portal. Any username and password created for our Patient Portal before this date will no longer work. You will need to follow the instructions below to create a new account for this new Patient Portal. All of your past data will still be available in this new Patient Portal.
1. Click on the “Create Account” tab.
2. Enter your name, date of birth, either the last 4 digits of your social security number or your medical record number and email address *Must be the one provided during your last visit to our facility* Click Submit.
3. You will then receive an email to the email address provided above with a link that says “Click here to sign in”. Click on this link.
4. You will be brought back to the Patient Portal login screen and a temporary username and password will be defaulted into those fields. Click on “Sign In”.
5. Follow the instructions on the next screens to complete your enrollment.
